NĐ 96/2023/NĐ-CP Phụ lục I Mẫu số 11 là mẫu Giấy xác nhận quá trình hành nghề, không phải mẫu GPHN; Phụ lục II Mẫu số 07 là bảng mã cơ quan cấp giấy phép hoạt động cơ sở KCB, không phải bảng mã số GPHN.

Từ 01/07/2026, NQ 21/2026/NQ-CP Điều 2 khoản 3 điểm a phân cấp cho Chủ tịch UBND cấp tỉnh các thủ tục GPHN trong đúng phạm vi đối tượng và điều khoản được liệt kê tại đó. Quy định này thay đổi thẩm quyền thủ tục, không tự thiết lập format số GPHN; Nghị quyết có hiệu lực đến 01/01/2028 và chịu điều kiện tại Điều 6 khoản 4.

Đây là định danh hành nghề hiện hành — SHOULD được populate cho nhân viên y tế hành nghề trong dữ liệu mới.

KHÔNG thuần lịch sử: NĐ 96/2023/NĐ-CP Điều 143 khoản 2 cho phép tiếp tục sử dụng như giấy phép hành nghề tới khi chuyển đổi khi làm thủ tục gia hạn, với phạm vi hành nghề đã được phê duyệt; Điều 143 khoản 3 đặt mốc áp thời hạn 05 năm từ 2030 và hết hiệu lực 2035. Trạng thái từng số CCHN đọc ở extension practiceLicenseStatus, không suy từ loại định danh.

Slicing tách hai trục khác nhau: RegulatedTitle là CHỨC DANH PHÁP LÝ quyết định nghĩa vụ giấy phép hành nghề (Luật 15/2023/QH15 Điều 26 khoản 1); Education là HỌC VỊ/BẰNG CẤP (BS, BS CKI, ThS, TS...) và không kích hoạt nghĩa vụ nào. Trước 0.10.0 hai trục này dồn chung một binding extensible vào VNQualificationVS, khiến không phân biệt được là bác sỹ với có bằng tiến sỹ.

Khai một trong 10 chức danh tại Luật 15/2023/QH15 Điều 26 khoản 1. Slice này KÍCH HOẠT invariant vn-practitioner-regulated-title-license. Cardinality 0..1 vì Điều 27 khoản 1 chỉ cấp 01 giấy phép hành nghề cho mỗi người; chức danh trên giấy phép là duy nhất.

A person who is directly or indirectly involved in the provisioning of healthcare.

The logical id of the resource, as used in the URL for the resource. Once assigned, this value never changes.

The only time that a resource does not have an id is when it is being submitted to the server using a create operation.

The metadata about the resource. This is content that is maintained by the infrastructure. Changes to the content might not always be associated with version changes to the resource.

A reference to a set of rules that were followed when the resource was constructed, and which must be understood when processing the content. Often, this is a reference to an implementation guide that defines the special rules along with other profiles etc.

A human language.

A human-readable narrative that contains a summary of the resource and can be used to represent the content of the resource to a human. The narrative need not encode all the structured data, but is required to contain sufficient detail to make it "clinically safe" for a human to just read the narrative. Resource definitions may define what content should be represented in the narrative to ensure clinical safety.

Contained resources do not have narrative. Resources that are not contained SHOULD have a narrative. In some cases, a resource may only have text with little or no additional discrete data (as long as all minOccurs=1 elements are satisfied). This may be necessary for data from legacy systems where information is captured as a "text blob" or where text is additionally entered raw or narrated and encoded information is added later.

These resources do not have an independent existence apart from the resource that contains them - they cannot be identified independently, and nor can they have their own independent transaction scope.

This should never be done when the content can be identified properly, as once identification is lost, it is extremely difficult (and context dependent) to restore it again. Contained resources may have profiles and tags In their meta elements, but SHALL NOT have security labels.

Khai căn cứ pháp lý khiến nghĩa vụ có giấy phép hành nghề KHÔNG áp dụng cho người này, theo Luật 15/2023/QH15 Điều 19 khoản 2, 3 và 4.

Vì sao là extension trên Practitioner chứ không phải một trạng thái credential. Những người thuộc diện miễn trừ thường KHÔNG CÓ credential nào để gắn trạng thái lên — sinh viên đang thực hành, nhân viên y tế thôn bản, người cấp cứu tại cộng đồng. Vì sao vẫn KHÔNG áp regex. Không phải vì sắp xếp đơn vị hành chính — giấy phép đã cấp vẫn còn hiệu lực nên mã cũ tiếp tục hợp lệ trên dữ liệu, và việc sáp nhập tỉnh chỉ làm các tỉnh đã sáp nhập ngừng cấp mới chứ không sinh mã cơ quan mới; tập 64 mã của Mẫu 03 là tập đóng.

Lý do thật: bảng Mẫu 03 chỉ có Bộ Y tế và 63 tỉnh, thành phố, thiếu Bộ Quốc phòng và Bộ Công an — hai cơ quan được Luật 15/2023/QH15 Điều 28 khoản 1 điểm b và c trao thẩm quyền cấp giấy phép hành nghề cho người làm việc tại cơ sở KCB thuộc thẩm quyền quản lý của mình. Ép regex theo bảng sẽ đánh trượt giấy phép hợp lệ do hai bộ đó cấp. Do đó profile bảo toàn nguyên văn giá trị và chỉ mô tả cấu trúc, không ràng buộc bằng máy.
